MEDICAL RECORDS SHOULD COME DIRECTLY TO ME FROM THE PROVIDER, NOT THROUGH THE PILOT

Safety assessments consider the validity of records through a chain of custody procedure. If the file comes through an non-medical intermediary, the file hypothetically could altered and this limits its usefulness.

Some medical records systems allow the client to print medical records at home. Not only would this break the chain of custody, most states do not allow a client to access mental health records without approval from the clinic.

After you have signed downloaded, reviewed, and signed the form, then you should contact the medical records office of your VA clinic or hospital to request your files.

You can also request records from the VA’s Centralized Support Division. These records take longer, often much longer, to arrive.

Compensation and pension files, also called the C-file, may require special attention. If you are requesting a C-file, please have a conversation with your VA medical records technician. If your compensation and pension assessment was conducted by a third-party agency you may need to file a separate form unique to your clinic.
